Club Sports Participation Requirements

All University of New England club-athletes are required to provide medical information prior to every year of participation in club sports, and must be cleared by the Athletic Training staff. The majority of this medical information is specific to club-athletes, and completely separate from what is required by the UNE Health Center for a student to attend the University. Any information provided is considered confidential and will be kept solely by the Athletic Training Staff.

~ NEW club-athletes must complete the following items prior participation:
1. 25-26 Participation Form (online)
2. SportsWare Information & Health History (online) [create an account - school ID code is "Noreasters", complete Insurance, Demographics, Emergency Contact, Health History]
3. 25-26 Risk Waiver (paper form) [complete PDF and upload to SportsWare under "attachments"]
4. Concussion Testing (you will complete in person with Nicole Driscoll, Club Sports ATC)
5. HIPAA Authorization (paper form) [complete PDF and upload to SportsWare under "attachments"]
6. Physical Exam Form (paper form) [complete PDF and upload to SportsWare under "attachments"] / must be dated by a physician within 12 months of the first day of team activity / cardiac section not required

~ RETURNING club-athletes must complete the following items prior to participation:
1. 25-26 Participation Form (online)
2. SportsWare Information & Health History (online) [Log in to your account and fill in Health History at the start of each year]
3. 25-26 Risk Waiver (paper form) [complete PDF and upload to SportsWare under "attachments"]
4. Concussion Testing (you will complete in person with Nicole Driscoll, Club Sports ATC)
5. HIPAA Authorization (paper form) [complete PDF and upload to SportsWare under "attachments"]
