Required Pre-Participation Forms & Documentation
In accordance with NCAA and Athletics Department policies, all University of New England student-athletes are required to provide medical information prior to every year of participation in pre-season and/or regular-season activities, and must be cleared by the Athletic Training staff. The majority of this medical information is specific to student-athletes, and completely separate from what is required by the UNE Health Center for a student to attend the University. Any information provided is considered confidential and will be kept solely by the Athletic Training Staff.
~ NEW student-athletes (incoming first-years/transfers) must complete the following items prior participation:
1. SportsWare Information & Health History (online) [create an account, complete Insurance, Demographics, Emergency Contact, Health History; school ID code is Noreasters]
2. Physical Exam Form (print PDF, have completed, and upload to SportsWare) [must be completed only on the UNE physical exam form, and must be dated by an MD, DO, or NP within 6 months of the first day of team activity]
3. HIPAA Authorization (print PDF, complete, and upload to SportsWare)
4. Sickle Cell Trait Documentation (copy and upload to SportsWare) [Hemoglobin S blood test, either from birth or recent test]
5. Sway Test (on campus)
6. (if applicable) ADD/ADHD Prescription Medication Report (upload to SportsWare) [from their prescribing physician, specifying need for medicine and treatment methods]
7. (if applicable) Asthma Letter (upload to SportsWare) [from their prescribing physician, referencing student name and date of birth, along with specifying need for medicine and medication, dosage, and if it is a steroid inhaler]
~ RETURNING student-athletes must complete the following items prior to participation:
1. SportsWare Information & Health History Updates (online) [update Insurance, Health History]
2. HIPAA Authorization (print PDF, complete, and upload to SportsWare)
3. Height, Weight, Blood Pressure, and Sway Test Updates (on campus)
4. (if applicable) ADD/ADHD Prescription Medication Report (upload to SportsWare) [must have a report from their physician on file, specifying need for medicine and treatment methods]
5. (if applicable) Asthma Letter (upload to SportsWare) [from their prescribing physician, referencing student name and date of birth, along with specifying need for medicine and medication, dosage, and if it is a steroid inhaler]
